EOSC is located on the first floor of the Elmhurst Memorial Center for Health. Patients and their families are encouraged to enter using Door 11, which is located on the north side of the building, and is closest to our suite. Please look for the signage directing you to “Surgery Registration.” |
Please sign-in at the front desk upon your arrival. Our registration staff will meet with you individually to fill out registration and insurance forms and sign an authorization for treatment. If you are under 18, your parent or guardian must be present to fill these documents out for you. Once registration is completed, you will be escorted into the pre-operative area where we will begin to prepare you for your procedure.
